What is the “Health Determinants” questionnaire?

A “Health determinants” questionnaire is sent to each patient undergoing treatment.

The purpose of this questionnaire is to collect data about the personal, social, economic or environmental factors which may influence your patients’ health status and to help you to assess their needs, particularly in terms of supportive care.

This questionnaire is an add-on the questionnaire for monitoring treatment-related side effects. 

It is generated automatically when the patient profile is created in Resilience and then every 6 months, and can be viewed in the mobile app.

What questions does the “Health determinants” questionnaire ask?

This questionnaire consists of 14 questions (plus 4 potential sub-questions) and addresses several themes:

  • Home support,

  • Professional situation,

  • Financial difficulties,

  • Level of studies,

  • Computer literacy,

  • Access to care,

  • Addictions, tobacco and alcohol,

  • Physical activity,

  • Understanding of the disease.

How can I view my patient’s answers?

This questionnaire does not generate an alert to the practitioner.

To view the answers to the questionnaires, go to the “Health report” tab of the patient file, then click on “Health determinants”.
